Your Rights Regarding Your Health Information

You have the following rights concerning your PHI:

1. Obtain a Copy of This Notice

You may request a paper copy of this Notice at any time.

2. Inspect and Copy Your PHI

You may request to review or obtain a copy of your PHI in electronic or paper format. Requests should be submitted in writing to Nu Health RX Privacy Office. Reasonable fees may apply for copies.

3. Request an Amendment

If you believe your PHI is incorrect, you may request an amendment with supporting documentation. We may deny requests if the records are accurate and complete or were not created by us.

4. Receive an Accounting of Disclosures

You may request a list of disclosures made outside of treatment, payment, or health care operations. The first request in a 12-month period is free; additional requests may incur a fee.

5. Request Confidential Communications

You may request that we contact you in a specific way or at an alternative location. We will accommodate reasonable requests.

6. Restrict Certain Uses and Disclosures

You may request additional restrictions on PHI disclosure. We are not required to agree, except when the disclosure is to a health plan for services you paid for in full out-of-pocket.

7. Notification of Breach

You have the right to be notified if there is a breach of your unsecured PHI.

How to Report a Privacy Concern
